TRIGLYCERIDES
Triglycerides (TG) are the chemical form in which most fats exist in food (both animal
and plant fats). They are also present in the blood along with cholesterol.
A diet that is high in fat, sugar, refi ned carbohydrates, and alcohol can elevate
TGs. Overeating also raises TG because excess calories are converted to fat in the
liver and then into TG to be transported in the blood. High levels of triglycerides are
associated with heart disease and diabetes. It is possible for triglycerides to be high
even when blood cholesterol is normal, so get your levels checked regularly. In most
cases, TG levels can be effectively managed with diet and exercise.
SUMMARY
In this section we learned that our bodies need a balance of quality protein, carbo-
hydrates, and fats. These macronutrients provide us with the energy and nutrients
needed for proper growth, development, and many body processes. In a later chapter
I will outline principles for a healthy diet—my top recommendations for a nutritional
plan for optimal health and disease prevention.

Vitamin E Is Anti-Cancer

Despite great advances in medicine, cancer is still a very common disease. At current rates, more than a third of us will suffer from cancer during our lifetime and about one in four of us will die from it. According to the American Cancer Society, 7.6 million people worldwide died from cancer in 2007.

Research statistics report that 30 to 40% of cancers are directly linked to nutrition, which shows the importance of a healthy diet in cancer prevention. Meanwhile, scientists continue to investigate foods known to be anti-carcinogenic in order to develop strategies for both cancer prevention (chemoprevention) and treatment (chemotherapy). As Hippocrates said over 2000 years ago, “Let food be thy medicine.”

The problem with current chemotherapeutic treatments is that most are not completely selective for carcinogenic cells—they harm normal cells too—and decrease the quality of life for cancer patients. Scientists are hoping that by developing chemopreventive or chemotherapeutic agents based on natural micronutrients, they will be able to develop drugs that target cancer cells with less or no harm to normal cells.

Vitamin E is an important antioxidant micronutrient. Research shows that vitamin E not only improves skin health, boosts the immune system, protects against heart disease, aging and Alzheimer’s, but also has a role in protection against some types of cancer. Unlike some vitamins, which consist of a single compound, vitamin E consists of eight different compounds, four tocopherols and four tocotrienols. Our food contains all eight compounds. Tocopherols are most commonly found in nuts and vegetable oils, whereas tocotrienols are primarily derived from palm oil, oat, rye, wheat germ, barley and rice bran.

Even though the vitamin E family consists of 8 members, most research has traditionally focused on alpha-tocopherol. Alpha-tocopherol is the most abundant form of vitamin E in the plasma and tissue of humans as well as in vitamin supplements, while the seven other types of vitamin E were greatly ignored for many years. Recent research over the last few years has led to the conclusion that the eight vitamin E compounds have different anti-carcinogenic potencies which need to be considered.

The cancer preventive properties of vitamin E were first suspected when studies showed that people in the Mediterranean area who consume diets rich in vitamin E have a lower risk of colon cancer than people in Northern Europe and the U.S. (Eur J Clin Nutr. 1989; 43:49-55, Cancer Causes Control. 1995; 6:525-31.) More recently, the Melbourne Colorectal Cancer Study showed that dietary vitamins E and C were protective for both colon and rectal cancer, and that for both vitamins there was a dose-response effect of increasing protection. (Nutr Cancer. 2006; 56:11-21.)

Another clinical study supported a preventive effect of vitamin E in the development of prostate cancer. This study included over 29,000 elderly male smokers and showed that those taking vitamin E for six years had 32% fewer diagnoses of prostate cancer and 41% fewer prostate cancer deaths than men who did not take vitamin E. (J Natl Cancer Inst. 1998; 90:440-446.) The ongoing NIH sponsored Selenium and Vitamin E Cancer Prevention Trial (SELECT) is investigating selenium and vitamin E for prostate cancer prevention based on the encouraging results of earlier studies.

In the past few years most of the research has focused on structural variations of the natural forms of vitamin E with the aim of developing the anti-carcinogenic potency of these agents. The scientific community is hoping to take some of the synthetic derivatives to clinical trials, so it will be interesting to see if any of these compounds will be eventually developed into chemopreventive or chemotherapeutic agents.

But you should be aware that all vitamin E supplements are NOT created equal. Many contain synthetic vitamin E, and many do not contain all of the various types of natural vitamin E that the body requires. Make certain that your supplement contains both natural tocopherols and tocotrienols in their natural forms if you want to get the most out of what vitamin E has to offer. Only products that contain the complete vitamin E family—tocopherols plus tocotrienols—provide the full spectrum of benefits of vitamim E.

The Anti-Aging Bottom Line: A careful examination of the research performed so far on vitamin E and cancer leads me to conclude that vitamin E possesses strong anti-carcinogenic properties. While food is by far the preferred source of vitamin E, it is practically impossible to get protective amounts of vitamin E from food alone. Therefore, I feel that taking supplemental vitamin E is absolutely essential to achieving vibrant health and longevity.

VITAMIN D - NOT JUST FOR BONES ANYMORE

The days are finally getting longer, but the cold and wind have kept us indoors through much of this winter. Even if we were outside on a regular basis, our skin would produce very little, if any, vitamin D. While the sun is closest to the earth in winter, the sun’s rays are entering at such an oblique angle (zenith angle) the solar ultraviolet B (UVB) rays are blocked by the ozone layer — and it is this solar UVB radiation that stimulates vitamin D production. But even in the summer, in the early morning and late afternoon, the zenith angle of the sun’s rays is so oblique that very little vitamin D is produced in the skin.

Why is this important? Vitamin D is recognized as the sunshine vitamin, and if we get little UVB radiation, we produce very little vitamin D. More than 90 percent of the vitamin D we need comes from casual exposure to sunlight.

But production decreases sharply with increased skin pigmentation (racial or seasonal), advancing age and use of sunscreens. Other important factors include the season and even the time of day. As a result, dark skinned individuals and the elderly may suffer from vitamin D deficiency, especially during the winter months.Vitamin D is assumed to be plentiful in a healthy diet but, in truth, very few foods naturally contain it and only a few are fortified with it. Moreover, recommended intakes of vitamin D are not sufficient to prevent deficiency. This, together with the widespread use of sunscreens and an overall lack of sunshine exposure, is the reason vitamin D deficiency is now epidemic in the United States.

Adults with vitamin D deficiency absorb calcium poorly and are at increased risk for osteoporosis and related fractures. If this deficiency is not corrected, extreme degrees of deficiency may ensue, leading to the painful bone disease of osteomalacia.

Other consequences of vitamin D deficiency include impaired neuromuscular function with poor balance, decreased muscular strength and increased falls. Individuals may complain of deep aching bone pain, weakness and muscular discomfort. Such patients are often misdiagnosed with fibromyalgia, chronic fatigue, myositis or other nonspecific conditions. It has been estimated that 40 to 60 percent of patients with fibromyalgia may have some component of vitamin D deficiency and osteomalacia. Vitamin D deficiencies have been demonstrated to increase the risks of many autoimmune diseases including Type 1 diabetes, rheumatoid arthritis, multiple sclerosis, inflammatory bowel disease and psoriasis. Another action of vitamin D is thought to be important in keeping cell growth in check, and preserving normal cellular proliferation and differentiation, thus preventing cells from developing into unregulated cancers. Deficiency of vitamin D is now felt to contribute to several major cancers, notably

prostate, colon and breast.

So what can be done to prevent vitamin D deficiency? First, it’s important to recognize that it exists, that it is epidemic and that it may affect you since no age group is immune. Even teenagers and children are at risk. Second, get sensible sun exposure — 5 to 10 minutes at a time, two to three times per week between the hours of 10 a.m. and 3 p.m. in the spring, summer, and fall. (It is equally important to avoid chronic, excessive exposure to sunlight which increases the risk of nonmelanoma skin cancers.)

Increased dietary and supplemental vitamin D intakes of 800 to 1000 international units (IU) per day are reasonable and safe.

Maintaining a normal vitamin D status should be a high priority for all of us, with the help of our doctor.

Safety of Multivitamins Questioned

The use of some vitamin supplements can be controversial, with scientists and manufacturers arguing over their usefulness. But they don’t spend much time arguing about the lowly and somewhat boring multivitamin.No one has ever argued that multivitamins might not be good for you. Until now.

The Harvard Men’s Health Watch, which once endorsed these popular supplements, now says that a reappraisal of that advice is in order.

The publication, in its March 2008 issue, notes that some recent studies have linked multivitamin use to prostate cancer. More convincingly, it says studies have linked high intakes of folic acid to colon polyps, the precursors of colorectal cancer.

Researchers speculate that high intakes of folic acid, which was first added to grain products in the 1990s, may have contributed to an increase in colorectal cancers in the mid-1990s.

What does all of this have to do with multivitamins?

Now that folic acid is added to so many grain products, it’s easy to see how a healthy diet, combined with a multivitamin, could boost a person’s daily intake to 1,000 mcg or more, potentially increasing the risk of colorectal and possibly prostate and breast cancers.

In light of this research, Harvard Men’s Health Watch editors suggest that the average man give up the multivitamin, at least until scientists solve the puzzle of folic acid and cancer.

However, if you stop taking a multivitamin, the authors suggest you consider taking a vitamin D supplement. The typical diet for most men and women doesn’t supply enough of this crucial vitamin, and while sun exposure boosts vitamin D production, it has health risks of its own.

Last month, a study suggested some multivitamins might increase cancer risk.

“Our study of supplemental multivitamins, vitamin C, vitamin E and folate did not show any evidence for a decreased risk of lung cancer,” wrote the study’s author, Christopher G. Slatore, M.D., of the University of Washington, in Seattle. “Indeed, increasing intake of supplemental vitamin E was associated with a slightly increased risk of lung cancer.”

Findings of the study of 77,000 vitamin users were published in the first issue for March of the American Thoracic Society’s American Journal of Respiratory and Critical Care Medicine.

Fresh fruits, vegetables and unprocessed grain products such as whole-grain breads and cereals are naturally low in fat, cholesterol-free, but rich in starches and dietary fiber. Including more starches and fiber in your diet can help you lower your cholesterol level as well as reduce your risk for obesity, cancer, high blood pressure and other maladies.
